The MSI-BPD includes 10 questions answered yes or no. Items relate to DSM-oriented borderline personality disorder criteria themes such as abandonment fears, unstable relationships, identity disturbance, impulsivity, self-harm risk themes, emotional reactivity, emptiness, anger, and stress-related paranoia or dissociation.

The cutoff of 7

In published adult samples, a total of seven or more “yes” answers is commonly treated as a positive screen—meaning clinical follow-up is recommended. Sensitivity and specificity figures are strong in research contexts, but self-administered online results still depend on honest answering and cannot replace an interview.

Screening versus diagnosis

A positive MSI-BPD screen is not a BPD diagnosis. Diagnosis requires a qualified mental health professional, fuller history, and consideration of other conditions. A negative screen also does not mean you are fine if you are suffering—seek help whenever distress is high.
